Job Summary: As a Private Duty Home Health Aide with Extended Hands, you will play a crucial role in providing essential care to our clients in the comfort of their homes. Your responsibilities will include assisting with personal care, household tasks, meal preparation, and providing companionship. This role requires empathy, excellent communication skills, and a dedication to enhancing the overall well-being of our clients.
Key Responsibilities:
Personal Care: Assist clients with bathing, dressing, grooming, toileting, and other activities of daily living (ADLs) as needed.
Mobility Support: Provide support with transfers, ambulation, and exercises as recommended by healthcare professionals.
Medication Reminders: Assist clients with medication reminders, ensuring they take their medications as prescribed.
Companionship: Engage in meaningful conversations, recreational activities, and provide emotional support to improve the client's social and emotional well-being.
Meal Preparation: Prepare and serve nutritious meals in accordance with dietary preferences and restrictions.
Household Assistance: Perform light housekeeping tasks such as cleaning, laundry, and organizing to maintain a safe and comfortable home environment.
Transportation: Assist with transportation to medical appointments and community outings, if required.
Documentation: Maintain accurate and thorough records of care provided, observations made, and any changes in the client's condition.
Communication: Collaborate effectively with the healthcare team, including nurses and therapists, to ensure coordinated care.
Qualifications:
High school diploma or equivalent.
Successful completion of a state-approved Home Health Aide (HHA) training program.
CPR and Basic Life Support (BLS) certification preferred.
Compassion, empathy, and a strong desire to provide high-quality care.
Excellent communication and interpersonal skills.
Ability to work both independently and as part of a care team.
Reliable transportation and a valid driver's license (if required).
Willingness to undergo background checks and drug screenings as mandated by state regulations.
