Job Summary: As a Private Duty Registered Nurse (RN) with Extended Hands, you will be responsible for providing one-on-one nursing care to patients in a home setting. Your primary role will be to assess patients, develop and implement care plans, administer medications, and offer support to patients and their families. This position requires excellent clinical skills, strong communication abilities, and a commitment to delivering compassionate care.

Key Responsibilities:

  1. Patient Assessment: Conduct comprehensive assessments of patients' physical, psychological, and social needs to develop individualized care plans.

  2. Medication Management: Administer medications, treatments, and procedures as prescribed by physicians, ensuring accuracy and safety.

  3. Monitoring: Continuously monitor patients' vital signs, symptoms, and overall condition, making adjustments to care plans as necessary.

  4. Wound Care: Provide wound care, dressing changes, and other specialized care as required.

  5. Education: Educate patients and their families on disease management, medication management, and preventive healthcare practices.

  6. Documentation: Maintain accurate and up-to-date patient records, including progress notes and care plans.

  7. Communication: Collaborate with the patient's healthcare team, including physicians, therapists, and other healthcare professionals, to ensure coordinated and holistic care.

  8. Emergency Response: Be prepared to respond to medical emergencies promptly and effectively.

  9. Comfort and Support: Offer emotional support and companionship to patients, helping to enhance their quality of life.

  10. Safety: Ensure a safe environment for patients, including fall prevention, infection control, and medication safety.

Qualifications:

  • Current state licensure as a Registered Nurse (RN).

  • CPR and Basic Life Support (BLS) certification.

  • Minimum of 1 year of nursing experience, preferably in a home healthcare or private duty setting.

  • Strong clinical assessment and critical thinking skills.

  • Excellent communication and interpersonal abilities.

  • Compassionate and patient-centered approach to care.

  • Ability to work independently and as part of a healthcare team.

  • Reliable transportation and a valid driver's license (if required).

  • Willingness to undergo background checks and drug screenings as mandated by state regulations.
