Job Summary: As a Private Duty Licensed Practical Nurse (LPN) with Extended Hands, you will play a crucial role in delivering high-quality care to our clients. Your primary responsibilities will include assisting registered nurses and physicians in providing care, administering medications, monitoring patients' conditions, and ensuring a safe and comfortable home environment. This position requires strong clinical skills, empathy, and a commitment to enhancing the well-being of our clients.
Key Responsibilities:
Patient Care: Provide direct patient care, including medication administration, wound care, and assistance with activities of daily living (ADLs).
Assessment: Assist in the assessment of patients' physical, emotional, and psychological needs, and communicate findings to the healthcare team.
Medication Management: Administer prescribed medications, document medication administration, and educate patients and families about proper medication management.
Monitoring: Continuously monitor patients' vital signs, symptoms, and overall condition, reporting any changes promptly.
Documentation: Maintain accurate and thorough patient records, including progress notes and care plans.
Safety: Ensure a safe and clean home environment for patients, identifying and addressing potential hazards.
Support: Offer emotional support and companionship to patients, promoting their physical and emotional well-being.
Communication: Collaborate with registered nurses, physicians, and other healthcare professionals to provide coordinated and comprehensive care.
Family Education: Educate patients and their families about care plans, disease management, and available resources.
Qualifications:
Current state licensure as a Licensed Practical Nurse (LPN).
CPR and Basic Life Support (BLS) certification.
Minimum of 1 year of nursing experience, with a preference for home healthcare or private duty experience.
Strong clinical and assessment skills.
Excellent communication and interpersonal abilities.
Compassionate and patient-centered approach to care.
Ability to work both independently and as part of a healthcare team.
Reliable transportation and a valid driver's license (if required).
Willingness to undergo background checks and drug screenings as mandated by state regulations.
